Privacy notice

This notice is published by Malama Care LLC, doing business as Telehealth Wellness Hawaii ("we", "us"), 91-1007 Waiilikahi Street, Ewa Beach, HI 96706. Questions go to telehealthwellnesshawaii@gmail.com or (917) 582-2356.

This notice covers this website, telehealthwellnesshawaii.com. It does not cover the care you receive as a patient. Health information you share once you become a patient is protected health information under HIPAA, is handled in our clinical systems rather than on this website, and is described in the Notice of Privacy Practices we give you when you join.

Please do not send health information through this site

The forms here collect contact details only. Please keep health details for your consultation, where we can take them securely. If you do include health details in a message, we treat them with care, but the website forms and email are not designed for them.

What we collect

• Consultation requests. Your name, email, phone, island, the topic you want to discuss and any message you type, so we can reach you and schedule your free consultation.
• Newsletter sign-ups. Your email address, so we can send the guides you asked for. Every email has an unsubscribe link.
• Payments. Membership payments are taken by Stripe on Stripe's own pages. We see your name, email, the amount and the last four digits of your card. We never see or store your full card number. Stripe's privacy policy applies to the details you enter there.
• Technical data. Our hosting provider, Cloudflare, logs requests to the site (IP address, browser type, pages requested, timestamps) to run the service and protect it from abuse. We do not combine these logs with your contact details.

What we do not collect

The BMI calculator runs entirely in your browser and sends nothing to us. We do not buy data about you, and we do not sell or rent personal information to anyone.

Cookies and storage on your device

This site sets no advertising or tracking cookies. It uses your browser's local storage for two things: to remember your cookie choice, and to remember small preferences such as a dismissed banner. These are essential to how the site works and are never shared.

If you allow it in the cookie banner, we may use privacy-friendly analytics to learn which pages help visitors most. Nothing optional loads unless you choose "Allow analytics", and you can change your mind at any time from the "Cookie settings" link in the footer. At the moment no analytics are installed, and the banner tells you so.

Fonts are loaded from Google Fonts. Your browser requests the font files from Google, which means Google receives your IP address and the page address in the same way as for any image loaded from another server. Google's privacy policy applies to that request.

How we use what you give us

• To answer your request, schedule your consultation and follow up about it.
• To send the newsletter you subscribed to, until you unsubscribe.
• To take your membership payment and send receipts.
• To keep the site secure and working.

We do not use your contact details for advertising on other platforms, and we do not send marketing texts unless you have asked for them.

Who we share it with

Only the services we need to run the practice and this site, each under its own contract: Cloudflare (hosting), Resend (delivery of form submissions and email), Stripe (payments) and Google (fonts). We may also share information when the law requires it. Multraverse, our technology partner, builds and maintains this site and our systems under a written agreement that limits what it may do with information.

How long we keep it

Consultation requests are kept for as long as needed to respond and, if you join, become part of your patient record. If you do not join, we delete them within 12 months. Newsletter addresses are kept until you unsubscribe. Payment records are kept as long as tax and accounting law requires.

Your choices and rights

• Ask us what contact details we hold about you, and ask us to correct or delete them.
• Unsubscribe from the newsletter with the link in any email, or by contacting us.
• Change your cookie choice at any time from "Cookie settings" in the footer.

Patient records are subject to HIPAA and Hawaiʻi law, which give you separate rights described in our Notice of Privacy Practices. Email us to use any of these rights and we will respond within 30 days.

Children

Our services are for adults. We do not knowingly collect information from anyone under 18 through this site.

Security

The site is served over HTTPS only, with security headers that limit what scripts can run. Form submissions travel encrypted to our email. No method is perfect; if you believe your information has been exposed, contact us straight away.

Changes

We will post any change here and update the effective date. A material change will also be announced on the home page for 30 days.
